Don't check for DEBUG before using dprintf.

The dprintf macro is no-op when DEBUG is unset, so there is no need to
put it conditional to DEBUG.

Remove offset_t typedef and use int64_t directly instead.
The name offset_t is easily confused with the standard off_t type and
*_t is POSIX reserved namespace if any POSIX header is included.

Globally rename the header inclusion guard names.

Consistently apply this rule: the guard name is obtained from the
filename by stripping the leading "lib", converting '/' and '.' to
'_' and uppercasing the resulting name. Guard names in the root
directory have to be prefixed by "FFMPEG_"....
