Replace FFmpeg with Libav in licence headers

Globally rename the header inclusion guard names.
Consistently apply this rule: the guard name is obtained from thefilename by stripping the leading "lib", converting '/' and '.' to'_' and uppercasing the resulting name. Guard names in the rootdirectory have to be prefixed by "FFMPEG_"....
Add FFMPEG_ prefix to all multiple inclusion guards.

md5 supportdepending on CONFIG_SMALL this can either be compiled to a fully unrolled kernel / rfc reference style md5 routineor a single loop similar to what mplayer uses
